Treatment of Osteoporosis
Reduced telomerase activity and shortened telomeres can lead to bone formation disorders, accelerate bone cell damage and bone mass loss, and the activation of telomerase can promote osteoblast differentiation. A study in 2020 found that CAG can up-regulate the expression level of TERT in bone damaged cells and increase alkaline phosphatase activity, thereby promoting osteoblast differentiation, while alleviating the inhibitory effect of DEX on osteoblast differentiation. Therefore, CAG can be used as a candidate for the treatment of glucocorticoid-associated osteoporosis (GIOP).

Idiopathic pulmonary fibrosis is an abnormal wound healing response after recurrent alveolar injury and is the most common manifestation of lung telomere dysfunction. In 2013, Le Saux et al. used a mouse model of idiopathic pulmonary fibrosis induced by bleomycin and found that CAG slowed down the aging of alveolar epithelial cells damaged by bleomycin by activating telomerase in lung tissue and extending telomere length, thus weakening the development of pulmonary fibrosis phenotype. Telomerase inhibitor GRN163L attenuates the action of CAG, suggesting that the inhibitory activity of CAG against idiopathic pulmonary fibrosis depends on its ability to activate telomerase.
Treating Neurodegenerative Diseases
Telomerase plays an important role in brain development and is also closely associated with neurodegenerative diseases. Telomere length has been used as a biomarker of cognitive decline in depression, schizophrenia, Alzheimer’s disease, and dementia. It has been found that CAG can induce the expression of TERT by activating the CREB signaling pathway in neurons, and play the function of telomerase activation, so as to realize the potential therapeutic effect on depression.
